Case Studies

Case #1
Resident developed swallowing difficulties referred for speech therapy and was placed on honey thick liquids. Other diagnosis included insulin dependent diabetic and Alzheimer’s disease. While living in a secured unit at a long term care facility, she ate and drank fluids well. She started to gain weight and her family complained due to purchasing new clothes. As glucose levels began to increase requiring more insulin, hemoglobin A1C went from 6.5% to over 9%. She frequently needed more insulin coverage due to higher glucose readings. Her fluid intake consisted of pre-thickened juice, milk and water. Other fluids were thickened with a powder. Her estimated carbohydrate intake from thickeners alone was approximately 110 grams per day. The facility switched her to Hydra Aid for all her fluids. The result: Her blood sugar elevations decreased her hemoglobin A1C improved to normal range and she began to have appropriate slow weight loss.

Case #2
Resident was admitted to a long-term care facility with a diagnosis of CVA and PEG tube for enteral feedings. With speech therapy, he passed a modified barium swallow test and began oral trials with thickened liquids and puree texture foods. The speech therapist started to work with his swallowing ability and RD introduced pureed foods with thickened liquids. He did well with the food – wanting to eat and the food tasted good. The liquids served - pre thickened and use of powdered thickener, were not acceptable and he refused fluids orally. He could eat sufficiently to be off the enteral feedings but could not meet hydration needs without his tube. His family wanted to take him home but knew the wife could not handle using the tube for hydration. Hydra Aid was introduced to the resident and family. He liked the taste much better and the family could easily prepare the fluids. He was able to take sufficient fluids to stop the tube use and he was able to discharge home.
